With 3dsDL (Scan QR Code). Then open the .zip in 3dsDL's Files and extract it where the release notes say.

About PicoDrive
The Mega Drive's FM sound runs on the 3DS's second CPU core, to keep the speed up on an Old 3DS. Sound on those models is 30000Hz, so high notes and samples can sound off. A New 3DS plays it at 44100Hz.
Mega CD games need the CD BIOS in sdmc:/3ds/picodrive_3ds/bios/, named bios_CD_U.bin, bios_CD_E.bin or bios_CD_J.bin by region. They run a bit slower on an Old 3DS, where a frameskip of one or two helps. 32X games are only playable with the CIA build on a New 3DS, and some, like Virtua Fighter and Virtua Racing Deluxe, crash.
There are save states, cheats and a region setting, and battery saves work for CD games too. The Mega Drive's A, B and C buttons sit on Y, B and A, and X, Y and Z on X, L and R. Tap the touch screen for the menu.
